INITIAL APPOINTMENT

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day. 

In preparation for your consultation, please ensure that you have the following information with you the day of your initial appointment:

  • The referral slip given to you by your dentist. To avoid misplacing or forgetting your referral slip, we strongly recommend that you or your dentist forward us a copy prior to your appointment date.

  • Any x-rays your dentist may have for you should be forwarded to our office in advance. If there is not enough time, please pick them up and bring them to our office. If additional images are necessary they can be taken at our facility at a cost.

  • A list of medications you are presently taking.

  • Your dental insurance information. Although we do not accept payment from your insurance company, we can help submit your dental claim on your behalf.

A preoperative consultation and physical examination is mandatory for patients undergoing IV anaesthesia for surgery. If you have been scheduled for consultation and treatment under IV anaesthesia on your initial visit, please alert the office if you have a medical condition (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, blood thinners (i.e. aspirin), anticoagulant therapy, etc.).


IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation and treatment appointment(s).
